That’s why we offer the Wellness Incentive—a way for you and your spouse (if applicable) to earn money for your Health Savings Account (HSA) simply by completing a FREE annual physical with routine lab work. No extra requirements. No health score to meet. Just a simple step toward better health and extra money in your pocket.

The 2026 Wellness Incentive uses a one-tier, completion-based model. You’ll earn the full amount by completing your annual physical with routine lab work—whether you’re on your own plan or covered with a spouse.
Here’s how it works:
Employee Only Plan
Complete a free annual physical
with routine lab work → Earn $600
Employee + Spouse Plan
- Employee completes a free annual physical
with routine lab work → Earn $300 - Spouse completes a free annual physical
with routine lab work → Earn $300 - Both complete a free annual physical
with routine lab work → Earn $600
Your annual physical is one of the best ways to stay ahead of potential health concerns. Even if you feel great, routine lab work and a conversation with your provider can uncover early warning signs and help you make adjustments before issues become serious. It’s a simple step that can make a big difference for your long-term health, and now, it’s also the only step you need to earn the full incentive.
This program is designed to be easier than ever to complete:

You receive the full incentive once your physical and routine lab work are done.
Dot remains committed to supporting the financial well-being of our employees and their families. That’s why we continue to deposit money into your HSA—an account that rolls over year to year, stays with you even if you leave Dot, and can be used for a variety of health care expenses now or down the road, including in retirement.
- Deadline to complete your physical and routine lab work: October 31, 2026
- Incentive payout date: January 2027—deposited directly into your HSA.
What’s the difference between an annual physical and a biometric screening?
Annual physicals and biometric screenings are both free and convenient ways to check in on your health.
An annual physical is a full review of your health. It includes a physical exam, a discussion of your medical history, and routine lab work. You’ll have time to talk through your results with your provider and build a plan that supports your long-term health and wellness goals.
A biometric screening is a quick snapshot. It usually includes a blood draw and checks key markers like blood pressure, cholesterol, and blood sugar. It’s helpful, but not as in-depth as an annual physical.
How does an annual physical help you take action?
An annual physical:
- Helps you and your provider create a personalized health plan based on your lab results, history, and any risks.
- Gives you a chance to check in year after year and track progress.
- Helps reduce future medical costs, lowers your chances of hospitalization, and supports better long-term health.
Where can I complete an annual physical?
Dot Family Health Center (FHC):
If you’re on Dot’s medical insurance, your FHC is the easiest place to complete your annual physical. Both the visit and lab work are free, and you don’t need to submit paperwork to earn the incentive.
A different primary care provider (PCP):
If you don’t have access to an FHC, you can see your own PCP. Just remember:
- Make sure both the provider and the lab are in-network to avoid extra costs.
- The physical and routine blood work must be coded as preventative, not diagnostic, to avoid additional fees.
- Your provider will need to complete a Physical Confirmation Form at the time of your annual physical/routine lab work.
- You will need to upload the document to your Marathon Health Portal once it’s completed by your provider:
- Log in or create an account at my.marathon.health
- Click “View Program Details” on the homepage
- Select the activity you are submitting a form for
- Click the “Upload Incentive Form” button to upload your completed document
- Once submitted, your form will be processed within 24-48 hours
What if I don’t have FHC access and don’t have a PCP?
Call your free Health Advocate at 855-608-7623 (listed on the back of your insurance card). They can help you:
- Find in-network providers accepting new patients
- Get your physical scheduled
Don’t wait. If you’re not already established with a PCP, it can take 6 months or more to get an appointment.
Can a DOT physical for Dot’s drivers be done at the same visit as an annual physical?
Yes. Drivers can complete both during the same appointment. A few additional items are covered during the annual physical, but the process is simple. When scheduling the DOT physical, drivers should mention they’d like to complete their annual physical for Dot’s wellness incentive at the same time.
Taking care of your health should be simple. Whether you visit a Dot Family Health Center or your own provider, you have free and convenient ways to complete your annual physical and earn your wellness incentive.
