Physician Assistant
Kyle has experience working in family
medicine, emergency, immediate
care and occupational medicine. He
believes that building trust by displaying
genuine care and compassion is key
to the provider-patient relationship.
Outside of work, Kyle enjoys
spending time with his wife and
5 kids as well as being an active
member in his local church.

Tracy Ribic
Licensed Practical Nurse
Tracy has worked with a wide range of
patients and specialties in her nearly
40 years as a nurse. Her experience in
oncology/hospice, pediatrics, and OB/
GYN has taught her the importance
of family and being present to the
people in your life. She finds great
joy in taking care of and interacting
with her patients and providers.
In her free time, Tracy loves playing
with her dog, gardening, and reading.

Free At The FHC
Annual Physical
What it is
A yearly exam that where you and your doctor will take a look at your overall health using routine lab work, screenings, and health assessments. This is your time to ask your provider questions and work together to build a plan for your health.
What you get:
Better health outcomes and lower health care cots. Your annual physical helps to catch diseases early, making them easier for your provider to manage and treat.

Condition Care Management
What it is:
Support from your provider to manage chronic conditions like high blood pressure, diabetes, high cholesterol, or heart disease
What you get:
Free visits, free lab work, and huge savings on medications to help manage your chronic condition.
Other Services
Everyone can benefit from having a primary care provider (PCP). That’s why anyone on Dot’s insurance over the age of two can use the FHC for:
General primary care
Allergy shots, vaccines, dermatology, lab services (blood draws), stitches, and more
Preventative care
Annual physicals, flu shots, skin checks, and well woman exams (all free!)
Sick visits
Treatment of common illnesses (strep throat, flu, sinus infections, etc.), including prescriptions and IV fluids.
Referrals
Help finding advanced care for things like a hospital procedure, MRI, x-ray, or specialist
